ZIP codes are more indicative of access to resources than genetic codes, highlighting challenges certain populations encounter to access healthcare. Post-pandemic, we are seeing tech innovations using Data Aggregation and AI to address these challenges of better meeting patients’ needs – one person at a time. Population health management provides a toolkit for healthcare leaders to understand common factors to transform care delivery systems and support front-line caregivers.
We will hear from Stephanie Ledesma on how the Path to Equity can be accelerated with Population Health Tools and how Kaiser Permanente supports the well-being of the communities they serve by leading improvements in health equity.
