Platforms have been a difficult sell in health care for many reasons – the industry is fragmented, it is complex, and highly regulated. The pricing model is opaque, and the economic incentives are misaligned. Care delivery is episodic, and the industry’s journey to technology adoption has been slow. The pandemic has illuminated the health inequities we face as a nation. ZIP codes are more indicative of access to resources than genetic codes, highlighting challenges certain populations encounter to access healthcare. Post-pandemic, we are seeing tech innovations using Data Aggregation and AI to address these challenges of better meeting patients’ needs – one person at a time. Population health management provides a toolkit for healthcare leaders to understand common factors to transform care delivery systems and support front-line caregivers.
We will hear from Stephanie Ledesma on how the Path to Equity can be accelerated with Population Health Tools and how Kaiser Permanente supports the well-being of the communities they serve by leading improvements in health equity.
